Fall 2008 Health Education Classes - Registration Form

  • Advanced registration is required. ALL fees must be paid at registration.
  • Classes are LIMITED IN SIZE and close when the maximum is reached.
  • You will NOT hear back from us if your registration is accepted.
  • If the class is filled or canceled, we will contact you and process your refund.
  • If a class does not meet minimum student registration, it will be canceled. Students will be contacted and fees refunded.
  • Other than classes canceled by the hospital, registration fees will be refunded ONLY if cancellations are made FIVE working days prior to class.
  • Refunds for a class with on-going sessions need a doctor's certificate if you drop out once the class begins.
Release

In consideration of participation in this program, I hereby indemnify, hold harmless, and release Whidbey General Hospital, its agents and employees from any and all liability, for injury suffered by myself arising from these classes, and I assume all risk for any injuries.
