Care managers work with your doctor to ensure the highest quality of patient care in the proper setting.

Medical social workers, and nurses assist you in planning your safe discharge home or another level of care. They can arrange home health care, a stay in a skilled nursing facility, transportation, and medical equipment you may need at home. They may help you with public assistance, insurance requirements for referrals needed at discharge, community resources for support groups, and outside agencies for drug and alcohol counseling.
