The Home Health Care multidisciplinary team provides an individualized approach to meet your health care needs in your home. Home Health cooperates with your physician to provide a variety of services including skilled nursing care, physical therapy, occupational therapy, speech therapy, medical social work services, and home health aide services depending on your condition and situation. The agency has state licensure for hospice services. The hospice team provides a team approach of specialized care designed to enhance the quality of life of the terminally ill. The team includes the above listed services, spiritual care, counseling and support, volunteers for respite and support, and bereavement services as appropriate.
