You will need to contact your automobile insurance to file an accident claim. Please contact our Financial Advocate at (360) 678-7656 or 321-7656, ext. 7601, within 10 days of your date of service, to complete the automobile insurance information we need to bill your insurance for you. If the information is not received within this time frame, we will make your account self pay.
